Overview
The Visiting Nurse Association of Florida (VNA) is a 50 year old home health agency that is growing and hiring Registered Nurses (RN) in the Hobe Sound area. As an RN, you will work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes. You will be responsible for completing physical assessments and determining patient home care needs, and assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model.
Responsibilities- Complete physical assessments and determine patient home care needs.
- Assist in developing and implementing plan of care for each patient using VNA of Florida's case management model.
- Collaborate with referral sources from the healthcare community to coordinate services in patients homes.
- Active Registered Nurse License required.
- Minimum 1 year clinical experience.
- Home Health/HCHB experience a plus.
- Trach/Vent and GT/Suprapubic – must have experience.
- Available to work every other weekend.
- Flexible scheduling and work/life balance.
- Independent practice.
- Supportive scheduling team.
- Variety of clients and diagnoses.
- Providing treatment and care to patients in their home.
