Wellness Supervisor LPN or LVN – Full‑Time

Vitality Living invites qualified LPN, LVN, or RN professionals to join our Baypoint Village Community as a Wellness Supervisor. This full‑time role focuses on coordinating resident care and staff support in a vibrant, inclusive environment.

Key Responsibilities

• Schedule and supervise staff to maximize resident care initiatives, ensuring accurate timekeeping and coverage during absences.
• Monitor, oversee and administer medications as permitted by state guidelines.
• Provide activities of daily living assistance, train new staff, and conduct ongoing in‑services.

• Promote positive employee relations, lead engagement and recognition initiatives to increase retention.
• Perform assessments and reassessments, and recommend appropriate service plans.
• Perform other duties as assigned.

Required Qualifications

• At least 21 years of age.
• Registered as an RN, LPN or LVN in good standing.
• Bachelor’s degree in health or human services or a related field, or equivalent education and experience.
• 2–5 years in a similar position within skilled nursing, home health, or rehabilitation settings serving older adults.
• Current professional license (RN, LPN or LVN), current CPR certification, and any other state‑required credentials.
• Excellent written and verbal English communication skills.
