- $39.93/hr midpoint (10+ years of experience)
- $43.92/hr maximum (15+ years of experience)
Shift Differentials:
- Evening Shift: +$2.50/hr
- Weekend Shift: +$2.00/hr Differentials stack; evening weekend shifts earn an additional $4.50/hr.

- One weekend required per 4-week schedule
Sign-On Bonus:
- $10,000 sign-on bonus paid over a 2-year commitment in five installments

Position OverviewA large acute care hospital is seeking an experienced Registered Nurse Case Manager to join its Case Management team. This role focuses on care coordination, utilization management, patient throughput, and discharge planning across the continuum of care. The RN Case Manager collaborates closely with interdisciplinary teams to ensure appropriate levels of care, optimal resource utilization, and high-quality patient outcomes.
The position reports to the Director of Case Management and plays a key leadership role in supporting efficient, patient-centered care delivery.
Key Responsibilities- Conduct comprehensive assessments of patients’ medical, psychosocial, and discharge needs
- Develop, document, and communicate individualized case management plans of care
- Lead interdisciplinary collaboration to ensure appropriate level of care and resource utilization
- Evaluate admissions and patient status for medical necessity using approved criteria
- Perform utilization management reviews and communicate findings to third-party payors
- Facilitate timely and appropriate discharge planning and post-discharge services
- Identify and address barriers to patient throughput and quality outcomes
- Act as a liaison among physicians, patients, families, hospital staff, and external agencies
- Ensure compliance with regulatory requirements, quality initiatives, and organizational policies
- Track trends, variances, and barriers to care and recommend process improvements
- Advocate for high-risk and underserved patient populations through education, referrals, and community resources
- Document case management activities and care coordination interventions accurately
- Participate in performance improvement initiatives and mandatory education
- Maintain regular, punctual, and dependable attendance
- Perform other duties as assigned
- Minimum of 3 years of recent case management experience in an acute care hospital setting
- Also considered: RNs with 3 years of acute care experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED
- Case management experience in home health or insurance may be considered with:
- 3 total years of acute care experience, and
- At least 1 year of acute care experience within the last 5 years
- Associate Degree in Nursing or Diploma in Nursing required
- Bachelor of Science in Nursing (BSN) preferred
- Active Florida RN license or valid compact license (Florida license required within 90 days of hire if compact)
- APRN license accepted if current and compliant
- Case Management, Nursing, or Utilization Review certification preferred

- Pay Details: $31.94 to $43.92 per hour.
