Base pay range: $1,000.00/daily - $1,200.00/daily.

About the Role

Aviata Health Group is looking for a Nurse Practitioner with Skilled Nursing Facility (SNF) experience to join our family. We value a compassionate approach and teamwork. Major Responsibilities
  • Serve as the SNF’s leading provider of internal medicine services on Saturday and Sunday.
  • Collaborate with the facility’s interdisciplinary team and physicians to reduce hospitalizations on weekends.
  • Work with the entire facility’s patient population, including short and long‑term residents, with assistance from nursing staff and medical directors.
  • Conduct new‑admission visits, discharge visits, and evaluations for patients with changes in condition.
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol for acute and chronic illnesses.
  • Prescribe all necessary medications and treatments.
  • Perform comprehensive physical examinations.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.

Qualifications
  • Advanced Practice Licensure (APN) with a Florida license.
  • Active license and DEA number.
  • Previous experience in nursing or related medical field.
  • Ability to build rapport with patients and their loved ones.
  • Capacity to thrive in a fast‑paced environment.
  • Excellent written and verbal communication skills.
  • Strong leadership experience emphasizing teamwork in a healthcare setting.
  • Master’s degree required.
