Physical Therapist (PT) – Skilled Nursing Facility (SNF) | Pahokee, FL – Just 45 minutes inland of West Palm Beach on beautiful Lake Okeechobee.
Responsibilities- Evaluate and treat patients with physical, emotional, or cognitive impairments.
- Develop individualized treatment plans and document progress in Net Health EMR.
- Collaborate with an interdisciplinary care team to promote optimal patient outcomes.
- Maintain 85% productivity.
- Active Florida PT license (or eligibility).
- Previous SNF experience preferred.
- Strong clinical and communication skills.
- Available to start ASAP.
