Key Responsibilities :
- Conduct thorough assessments to evaluate speech, language, communication, and swallowing disorders.
- Develop and implement individualized treatment plans to address speech, language, and swallowing difficulties.
- Provide direct therapy and interventions to patients in their homes, focusing on improving communication and swallowing functions.
- Educate patients, families, and caregivers on strategies to manage and improve communication and swallowing abilities.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care and optimal patient outcomes.
- Monitor patient progress and make necessary adjustments to treatment plans to achieve therapy goals.
- Document patient evaluations, treatments, and progress in the electronic medical record (EMR) system.
- Maintain compliance with agency policies, procedures, and state/federal regulations related to speech therapy in home health.
- Active and unrestricted Speech-Language Pathologist (SLP) license in Florida.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
- Minimum of 1 year of clinical experience, preferably in home health, rehabilitation, or a related field.
- Strong understanding of speech-language pathology practices and home health care regulations.
- Proficiency in electronic medical record (EMR) systems.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and manage multiple patients efficiently.
- Compassionate and patient-centered approach to care.
- Valid driver’s license and reliable transportation.
