Overview
Summary This HUD/VASH Social Work position is located within the Social Work Service of the Bay Pines VA Healthcare System - FL. The incumbent will provide services at the Bay Pines clinic. Housing and Urban Development/Veterans Affairs Supported Housing (HUD/VASH) Program is an intensive clinical case management program for homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's housing choice voucher system.
Responsibilities- The incumbent provides services to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice

- Duties of the Social Worker - HUD/VASH program include but are not limited to the following:
Independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs - The incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups in the outpatient mental health clinic
- Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members
- Independently assess the psychosocial functioning and needs of individuals and families from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds and is able to formulate and implement a treatment plan, identifying the patient's problems, strengths, needs, abilities, preferences, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team
- Offer psychosocial interventions and measure outcomes to interventions
- Work with patients and families who are experiencing emotional, medical and social problems, utilizing individual, group and family counseling skills
- Assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan
- Communicate effectively, both orally and in writing, with people from varied backgrounds
- Establish and maintain effective working relationships with clients, staff, and representatives of community agencies
- Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs
- Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills
- Identifies high-risk patients and provides case management services
- Work Schedule:
Monday - Friday, 8:00 am - 4:30 pm - Travel up to 75% of the time;
Valid Driver's License required
- Basic Requirements United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy - English Language Proficiency Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C.
- Education:
Have a master's degree in social work from a school fully accredited by the Council on Social Work Education (CSWE); graduates of candidacy-status schools do not meet this requirement until fully accredited - A doctoral degree in social work may NOT be substituted for the master's degree in social work
- Licensure:
Must be licensed or certified by a state to independently practice social work at the master's degree level; exceptions may apply per VA policy (GS-9 level) with specified timelines; GS-11 requires licensure or certification at independent practice level - Verification of degree available at http://(Use the "Apply for this Job" box below). state…
