Key responsibilities include:

• Evaluates the patient & home health environment to determine required services
• Serves as the patient’s advocate by providing information
• Responsible for timely completion & synchronization of OASIS visits as required & developing the POC based on findings from evaluation visit
• Involves patient & patient’s family in developing the plan of care

We seek the following principal qualifications:

• RN state licensure in good standing
• Bachelor or Masters level degree in Psych & experience in mental health
• Medicare, home healthcare, and OASIS experience
• 1 year of Psych RN experience within the last 2 years
