Specific Responsibilities

The incumbent, an employee of the Citrus County Health Department, represents the agency and its mission and values. The incumbent is part of the global public health organization and is expected to promote cooperation, courtesy and teamwork, in a diverse environment. This position is responsible for implementing the Special Supplemental Nutrition Program for Women, Infants and Children (WIC) at different clinic sites throughout the county as directed by the Nutrition Program Director.

Provide nutrition assessment, nutrition education, breastfeeding education and counseling, including high risk care plans, when appropriate, to clients enrolled in the Special Supplemental Nutrition Program for Women, Infants and Children (WIC). This position requires independent judgement based upon information found in the DHM 150-24 Policy and Procedure Manual for the WIC Program and the FL-WiSE Clinic Manual, FL-Wise Training Manual and Modules, including WiSE Notes, FL-WiSE Help Sheets.

Performs and assess anthropometric, medical, and dietary data to determine client’s eligibility for the WIC program in accordance with policies of the State WIC program and regulations stated in DHM-150-24. Perform anthropometric and hemoglobin measurements for clients during WIC certification, recertification, mid-certification, and follow ups when needed. Certifies and recertifies clients for the WIC program, including nutrition assessment and assignment of nutrition risk factors base upon procedures in the DHM 150-24 Policy and Procedure Manual.

Prescribes and makes changes in WIC food packages for clients.

Provides nutrition counseling to WIC participants and promotes nutrition/education goal setting pertinent to the needs of the client.

Document services in FL WISE using Subjective, Objective, Assessment and Plan (SOAP) charting method. Develop, monitor, evaluate and revise nutrition care plans for individual clients at high nutritional risk. Monitor and follow protocols for nutrition assessment and follow up for high risk WIC participants. Develops care plans and follows up with medically high‑risk clients in providing medical nutrition therapy counseling according to guidelines set up by Academy of Nutrition and Dietetics and as stated in DHM-150-24.

Discusses client’s progress with other professionals. Assess medical documentation forms to determine need for special formula or non‑contract formula following the guidelines stated in DHM-150-24. Provide referral to other health care and social service programs. Coordinate services with other CHD programs, including immunizations, primary care, Healthy Start, etc.…
