Job DescriptionKVC is seeking a Family Care Coordinator / Case Manager to work directly with families on a day-to-day basis while also managing and coordinating client care. The Family Care Coordinator / Case Manager will work with the Family Preservation and Reunification Services contracted by the state.
Requirements for Family Care Coordinator / Case Manager- Bachelors in Social Work (or other related field)

- Establish initial contact with families within 24 hours of referral to assess safety,
- Initiate safety planning, and connect families with resources to meet immediate needs;
- Conduct team meetings and maintaining communication with the family and providers
- Facilitating the implementation of each family’s Action Plan
- Make frequent and consistent contact with DCBS personnel
- Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
