Position Details
  • Salary: $39.00-$46.50 based on experience
  • Schedule:

    M-F 8a-5p, occasional evenings and/or weekends possible

The Registered Nurse (RN) provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant’s condition to primary caregivers, physician and care team.

Qualifications
  • Current licensure in the state of Kentucky as a Registered Nurse.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Current CPR Certification.
  • A minimum of one-year (1) experience in working with the frail or elderly population.
WORK ENVIRONMENT
  • May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and any other conditions common in a healthcare environment.
  • Subject to unpleasant odors.
Essential Functions
  • When requested by the Director of Clinical Services (DCS/RN), functions as a member of the Interdisciplinary Team, maintaining attendance at and participating in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.
  • In coordination with the DCS and other clinical staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living at home, in community-based settings, in a SNF or within residential facilities and coordinates care of participant with the and Interdisciplinary Team.

    Provides skilled nursing care within the PACE clinic and during in-home visits care.
  • Conducts a comprehensive initial nursing assessment with new PACE enrollees. If and when directed by the Clinic Manager and DCS, coordinated care with the Interdisciplinary Team, to develop a comprehensive plan of care for each participant. Conducts nursing re‑assessment semi‑annually if directed/when needed.
  • Performs general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Clinical Care/Interdisciplinary Team. Observes, records, and reports participant’s condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.
  • Participates in nursing on‑call rotation to provide after‑hours and weekend assistance and care, as needed, to PACE participants.
  • Receives participant complaints, triages, and responds to participant needs.
  • Reviews and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.
