Pay: $19.40 - $24.50 (based on experience)
Schedule:
M-F 8am - 5pm, occasional evenings and/or weekends possible
The Home Care CNA is responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of State (SCC). As assigned, works shifts in the SCC Day Center and in participant homes and provides care and services to participants and assists with the provision of a safe and effective day program.
Required Qualifications- Active and up-to-date CPR certification.
- Current STNA license.
- A minimum of one year experience working with the frail or elderly population.
- High School diploma or equivalent.
- Medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
- Valid driver’s license, proof of insurance and means of transportation; you will be traveling to participant homes.
- Bilingual skills are a plus.
- Consult with the Home Care Coordinator daily to receive updates on participant status and plan of care and to confirm scheduled visits.
- Assist participants in the home with activities of daily living (personal hygiene, ambulation, meals, medications normally self-administered) and follow assigned care plans.
- Support day center activities, maintain clean and orderly day rooms and assist with meal service and basic household tasks.
- Assist drivers with loading and unloading participants from vehicles as needed.
