About the Job

Pay: $19 - $24 (based on experience)

The Home Care CNA is responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of State (SCC). As assigned, works shifts in the SCC Day Center and in participant homes and provides care and services to participants and assists with the provision of a safe and effective day program.

Required Qualifications
  • Must have an active and up-to-date certification in CPR
  • Must have current STNA license
  • Minimum of one year experience working with the frail or elderly population
  • High School diploma or equivalent required
  • Must have medical clearance for communicable diseases and up-to-date immunizations before direct participant contact
  • Must have a valid driver’s license, proof of insurance and means of transportation; traveling to participant homes
Preferred Qualifications
  • Bilingual skills are a plus
  • Consults each day with the Home Care Coordinator (or designee) to be updated on changes in participant status, changes in plan of care, and to confirm scheduled visits
  • Assists participants in the home with the following duties:
    • Personal hygiene such as skin, mouth, hair care and bathing
    • Assists participants in and out of bed and with ambulation
    • Assists with prescribed exercise taught by appropriate personnel
    • Prepares meals, including therapeutic diets, and assists with meals
    • Assists participants to the bathroom or using commodes, bedpans, or urinals
    • Assists with medications ordinarily self-administered
    • Other duties as taught by a health professional for a specific participant such as vitals, colostomy bag changes, non-sterile dressing, and bowel/bladder hygiene care
  • Performs directed household/homemaker services
  • Assists participants and/or supervises in daily living activities in the Day Center as assigned:
    • Bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care, and toileting
    • Monitors and assists participants with meals at the feeding table
    • Maintains clean and orderly day rooms, kitchen and personal care areas
    • Performs or assists with laundry, including clothes for participants
    • Assists drivers with loading and unloading participants from vehicles
