POSITION SUMMARY

The Substance Use Counselor is responsible and accountable for the delivery of contracted substance use disorder services in the facilities within the Navajo County Detention Center. Primary duties include the screening and assessment of substance use disorders, assessment of appropriateness for residential treatment and implementation/oversight of a residential substance use program. The Substance Use Counselor will also work with designated Navajo County Reentry Staff to assist in coordination of aftercare services in the community.

DUTIES/RESPONSIBILITIES

Reviews intake screening of patients and assesses potential readiness and appropriateness for referral to the residential substance use disorder (SUD) program. Works with county staff to determine placement into the SUD program.

Reviews referrals received from healthcare providers, medical staff, programs staff and security staff to determine severity of need.

Completes screening tools for particular substance use disorders as needed.

Conducts assessment with referred patients to gauge the severity of an individual’s substance use and provides a comprehensive overview of an individual’s addiction‑related issues.

Provides group therapy services focused on treatment and management of substance use disorders.

Coordinates the SUD Program within the Navajo County Detention Center. Ensures all tasks associated with continuity and induction MOUD services are effectively scheduled and completed:

Screening
Consent for participation
Medical clearance
Lab work and drug screening
Inmate‑Patient readiness assessment
Group therapy
Individual therapy, as needed
Personal or self‑help study
Coordination of community referrals for coordination of care in concert with designated Navajo County Reentry Staff

Coordinates care with medical, psychiatric, and mental health providers to ensure treatment of comorbid conditions.

Coordinates with Navajo County staff to review admission into the program, adequate participation, and successful discharge.

Coordinates care with community providers and community supervision agencies, including the exchange of treatment records, communication of treatment needs and linkage of individuals with available services and support options upon release in concert with designated Navajo County Reentry Staff.

Obtains patient signatures on any refusals for SUD services.

Communicates with individuals via request, triages the requests and schedules tasks within the electronic health record.

Coordinates discharge planning with designated Navajo County Reentry Staff.

Ensures and maintains the confidentiality of all medical records information and patient care information.

Assists in preparing periodic statistical reports and other management information system requirements.

Understands and complies with all state, facility and company policies, procedures and protocols related to the substance use program.

Meets timelines related to case management and departmental productivity guidelines.

JOB REQUIREMENTS

LICENSING: Currently licensed as Licensed Independent Substance Abuse Counselor (LISAC), Licensed Associate Substance Abuse Counselor (LASAC), or Licensed Substance Abuse Technician (LSAT). LISAC preferred.
CERTIFICATION: Current CPR certification.
EDUCATION: Master’s Degree in Social Work, Counseling or related field.
PREFERRED EXPERIENCE:
- Minimum of two (2) years addictions treatment counseling preferred.
- Minimum of one (1) year correctional experience preferred.
