Private Duty Nurse (RN/LPN) – 1:1 Home Care

Location: Clio/Flint/Saginaw/Millington/Lapeer

Setting: In-home, one-on-one care

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

• Saginaw (1‑Year‑Old Baby Girl):
  History of HLHS and bronchomalacia. Clinical care includes Trach, Vent, and G‑Tube management. No pets in the home. (Open to Male or Female nurses; no Meet & Greet required).
  • Shifts:
    
    7:00 AM – 7:00 PM (Days) OR 7:00 PM – 7:00 AM (Nights).
  • Availability:
    Sunday through Saturday (All days available)
• Millington (4‑Year‑Old Boy):
  Playful child who loves toys and TV. Trach/G‑Tube care.
  • Shifts:
    
    3 PM – 7 PM (M–Thu) or 7 AM – 3 PM (Fri–Sat) (Female Nurse per family request)
• Clio (2.5‑Year‑Old Boy):
  Sweet toddler who loves books. Trach/ Vent/ G‑Tube care.
  • Shifts:
    
    7 PM – 7 AM (Sun, Mon, Wed, Sat)
• Clio (8‑Year‑Old Boy):
  Active and playful! Likes tag and video games. Trach/ Vent/ G‑Tube care.
  • Shifts:
    
    7 AM – 5 PM (M–F). Requires ability to lift 60 lbs.
• Flint (10‑Year‑Old Boy):
  High‑energy Spiderman fan! Trach care in a clean, pet‑free home.
  • Shifts:
    
    6:30 PM – 6:00 AM (Daily) or 2 PM – 2 AM (Sat/Sun)

• Lapeer (39‑Year‑Old Male):
  Specialized care (Trach/Vent/G‑Tube). Relaxed home with friendly dogs.
  • Shifts:
    
    Days (6 AM – 7 PM, Fri–Sun) or Nights (9 PM – 6 AM, Sun–Thu)
• Clio (56‑Year‑Old Male):
  Quadriplegic care including wound care and bowel program. Requires a dedicated caregiver to assist with appointments.
  • Shifts:
    
    6 AM – 4 PM (Tue & Thu) (Male Nurse per patient request)

• Provide skilled nursing care (trach/vent care, medication administration, catheter care)
• Monitor and document patient condition and vital signs
• Assist with daily living activities (ADLs)
• Communicate with families and care teams
• Maintain safe, high‑quality, patient‑centered care

• Full‑time, part‑time, and per diem shifts available

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills

• Pediatrics (birth–21 years)
