The Family & Community Outreach Coordinator should have a basic knowledge of the role and purpose of nonprofit organizations and have a clear understanding of the agency’s mission and work. The Program Coordinator must be someone who will bring awareness and appreciation of the organization and its programs to the community Family & Community Outreach Coordinator should be able to engage with others in a collaborative and open-minded way, ensuring confidentiality, to ensure work is completed and objectives are met.
The following describes the general nature of work to be performed but is not an exhaustive list. The Board of Directors reserves the right to revise the duties and responsibilities of this position at any time at its sole discretion.
This position is grant-funded under a contract with a partner agency and therefore may be subject to annual funding decisions.
Duties and Responsibilities Client Recruitment and Retention- Works on recruiting and retaining clients of Genesee Health Plan, and the Motherly Intercession RACS+ Program and Strengthening Incarcerated Families (SIF)
- Refers clients who qualify for GHP to ensure they have health care coverage
- Raises awareness about the availability and scope of Motherly Intercession programs; meeting with clients in the jail as well as potential referrals and caregivers who are not incarcerated
- Help facilitate the Parenting Education Services weekly groups.
- Create surveys to evaluate and get feedback about Parenting Education lessons and group sessions.
- Assists potential clients in completing forms and applications as needed.
- Bust on the strengths of clients, families, and communities to provide a safe and loving environment.
- Assist clients and families with a variety of issues, gathering relevant information about their situation. Gathers information over the phone and through in-person interviews.
- Works with the program staff and clients to establish short and long-term goals.
- Works across disciplines, with the agency and throughout the community, to help clients achieve their goals, strengthen families and keep children safe.
- Assists in crisis intervention, urgent referrals to other agencies and resources and ongoing case management.
- Provides parenting classes at the agency and at the jail or other off-site location
- Serves as a liaison with parents, caregivers, jail staff, counselors, and other agency professionals.
- The Family & Community Outreach Coordinator will work with all staff to advocate and promote the agency and its programs.
- Responsible for communications and outreach to various agencies, organizations and individuals to recruit and coordinate care for clients who could benefit from agency programs and services.
- When interacting with clients and the community, they serve as an ambassador and advocate for the agency including attendance at community events.
- Following up with individuals who express an interest in services or programs.
- Maintains data and demographic information on clients and potential clients. Analyzes and reports information as requested consistent with confidentiality requirements.
- Adds new applications (students/families/caregivers) in master files.
- Tracks program attendance, and survey results. Maintain files and reports as requested.
- Creates process notes for Genesee Health Plan and Parenting Education services.
- Responsible for providing information, and recommendations as requested, to the Deputy Director and others as directed.
- Works to achieve annual organizational and individual goals and objectives as established by the agency.
- Comply with the Personnel Policies and administer operational policies.
- Maintains records and prepares reports as requested.
- Provides support and assistance to other staff as requested.
- Comply with programming policies and guidelines. Reviews and follows the Employee Handbook and if additional clarification is needed, takes the initiative to ask for additional information or direction.
