Care Coordination – Social Worker (Per Diem)

Location:

HHS Hunterdon Healthcare System Inc., 2100 Wescott Drive, Flemington, NJ.

Schedule:

One weekend a month and an occasional weekday, Day shift.

Compensation: $40.00 Hourly.

Position Summary

Provide social work services to patients referred for social work assessment and intervention within assigned areas or programs. Provide short‑term therapeutic interventions to patients and families and assist with discharge planning and case management of hospitalized patients. Collaborate with other departments to problem‑solve issues.

Primary Responsibilities

• Facilitate and/or assist in creation, carryout, and completion of appropriate discharge plan.
• Document services provided.
• Provide emotional support to patients and families as they cope with anxiety, depression, or other issues related to their diagnosis and treatment.
• Oncology navigation/assessment of needs.

Qualifications Minimum Education: Master’s Degree in Social Work.
Required Experience: 1‑2 years graduate school or post‑graduate social work experience in an acute care hospital, ambulatory clinic and/or home care setting.
Preferred Experience: 2‑3 years post‑graduate clinical social work experience in an acute care hospital, ambulatory clinic and/or home care setting with at least one year of medical social work experience preferred. Experience in area position is located.
License: NJ Licensed Social Worker.
Knowledge, Skills and Abilities: Excellent writing, interpersonal and communication skills.
