Band 5 Community Mental Health Practitioner

Responsibilities

• Actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and code of conduct.
• Undertake mental health assessments including risk assessments within a multi-disciplinary assessment team.
• Be proficient in referring to other clinicians within the service or to other services/agencies where necessary.
• Provide a fast, responsive service to assess individuals' needs.
• Establish a communication structure between GP's, IAPT and non-IAPT services in LSCFT, ensuring the highest possible standards of patient care.

• Working knowledge of assessments, care planning and risk assessment and management of people under community services.
• Ability to explore issues relating to the impact of health and social needs on mental health and well-being.
• Experience as a newly qualified nurse or a student due to graduate.
