Responsibilities
- Actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and code of conduct.
- Undertake mental health assessments including risk assessments within a multi-disciplinary assessment team.
- Be proficient in referring to other clinicians within the service or to other services/agencies where necessary.
- Provide a fast, responsive service to assess individuals' needs.
- Establish a communication structure between GP's, IAPT and non-IAPT services in LSCFT, ensuring the highest possible standards of patient care.
- Working knowledge of assessments, care planning and risk assessment and management of people under community services.
- Ability to explore issues relating to the impact of health and social needs on mental health and well-being.
- Experience as a newly qualified nurse or a student due to graduate.

- Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
- Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
- Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long-term condition management.
