We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to serve as a Quality Assurance Nurse in Flagstaff.

This role is responsible for ensuring quality assurance, infection prevention, and compliance with all state, federal, and VA standards. The ideal candidate is detail-oriented, knowledgeable in healthcare compliance, and passionate about improving patient care through effective quality programs.

Responsibilities
  • Implement and monitor quality assurance policies and infection control programs.
  • Conduct audits, data analysis, and compliance reviews across departments.
  • Review CASPER reports and develop QAPI (Quality Assurance Performance Improvement) plans.
  • Prepare and interpret data summaries and statistical reports.
  • Assist leadership with corrective action plans and compliance monitoring.
  • Lead Quality Assurance and Infection Control Committee meetings.
  • Coordinate readiness for regulatory surveys and inspections.
  • Investigate incidents of non-compliance, abuse, or neglect.
  • Develop and deliver staff education programs related to quality and infection control.
  • Track and trend infections using McGeer and non-McGeer criteria.
  • Stay current with CMS, CDC, and healthcare regulatory standards.
Knowledge, Skills & Abilities Knowledge
  • Quality assurance and infection prevention standards (OBRA, CMS, VA).
  • Data gathering and performance improvement methodologies.
  • Nursing home regulations and staff training best practices.
Skills
  • Problem solving and analytical thinking.
  • Data interpretation and report generation.
Ability
  • Develop and coordinate effective training programs.
  • Work collaboratively with staff at all levels.
  • Analyze, evaluate, and implement compliance improvements.
Required Qualifications
  • Active LPN or RN license in the State of Arizona.
  • CPR/BLS Certification.
  • 2-Step TB Test (current).
  • Valid AZ Driver’s License (in-state travel may be required).
  • Must pass background, OIG/SAM clearance, and drug screening.
  • Local candidates only – must be available for in-person interviews and ready to start within 2 weeks of offer.
