• Evaluates the patient by performing chart review, interpreting tests and measurements appropriate to discipline and patient diagnosis.
• Performs treatments and patient care activities in a timely manner following appropriate protocols and guidelines.
• Participates in patient care conferences with other related health services to review progress and evaluate goals.
• Develops appropriate plan of care that includes goals and home instructions in conjunction with other team members, patients and families.
• Documents specific measures to meet assessed learning needs and documents in patient record.
• Implements the treatment plan and evaluates and records the patient responses to the treatment sessions.
• Participates in the development of individual discharge plans that are responsive to patient/family concerns.

• Bachelor's degree – Required
• Master's degree – Preferred
• Current Physical Therapist or Occupational Therapist in the state of Arizona – Required
• Certified Hand Therapist (CHT) – Required
• BLS (American Heart Association) – Required upon date of hire
• Ability to obtain and maintain appropriate clinical competencies for the roles assigned by departmental leader

Experience:

Competent in using computers, basic software programs, and effective communication with patients and team.
