Quality Assurance Nurse (RN / LPN) – Healthcare
Contract Duration: 6+ Months (Extendable)
Schedule: Full-Time | Day Shift | Monday–Friday (Varied weekends as needed)
Start Time Options: 7:00 AM–3:00 PM | 8:00 AM–4:00 PM | 8:00 AM–5:00 PM

Position OverviewWe are seeking a Quality Assurance Nurse (RN or LPN) to support Quality Assurance, Staff Education, and Infection Prevention initiatives at a healthcare facility in Flagstaff, AZ. This role ensures compliance with state, federal, and VA regulations while driving continuous quality improvement and infection control practices.
Key Responsibilities Quality Assurance & Compliance- Implement and monitor quality assurance policies and procedures in alignment with nursing home regulations, evidence-based practices, and facility standards
- Conduct audits, interviews, and observations to assess compliance across departments
- Review monthly CASPER reports and develop QAPI Performance Improvement Plans (PIPs)
- Prepare and distribute compliance reports, data analyses, and performance summaries
- Assist leadership in developing and implementing corrective action plans
- Monitor and report progress on correction of deficiencies identified by regulatory agencies
- Coordinate facility preparation for regulatory surveys
- Investigate incidents of non-compliance, including abuse, neglect, or mistreatment allegations
- Serve as Infection Preventionist for the facility
- Conduct daily infection control surveillance rounds
- Track, trend, and map infections using McGeer and non-McGeer criteria
- Chair and coordinate Infection Control Committee meetings
- Stay current with CMS, CDC, and regulatory guidance
- Identify training needs and develop quality assurance and compliance education programs
- Deliver staff education and re-education sessions
- Evaluate training effectiveness using assessments and competency-based protocols
- Maintain training and compliance documentation
- Active Arizona RN or LPN License
- DPS Level 1 Fingerprint Clearance Card
- CPR/BLS Certification
- 2-Step TB Test
- Hepatitis B Vaccination or Declination
- Quality assurance and utilization review principles
- OBRA and nursing home regulations
- Data analysis and statistical reporting
- Adult learning and competency-based training
- Analytical and problem-solving skills
- Ability to work collaboratively across all levels of staff
- Experience developing and coordinating training programs
- Start date dependent on interview completion, onboarding, and PO issuance
- Candidates must be able to start within 2 weeks of offer acceptance
