The Lead Care Coordinator is a member of the multidisciplinary team that actively facilitates patient flow during acute care episodes, linking patients to internal and external resources and the payor community. The coordinator leads the process of identifying complex outlier patients and plans, implements, monitors, and measures the effectiveness of more complex interventions to meet patients’ treatment and transitional needs.
ResponsibilitiesCare Coordination/Transition Planning
- Proactively screen and assess the acuity and transitional needs of the patient and support the Care Management team in their work with complex and outlier patients.
- Partners with the Interdisciplinary team to facilitate placement in a rehab, SNF, sub‑acute, assisted living facility, or home with Home Health Care, and arrange DME and other services as ordered.
- Identify appropriate consultative services that would enhance the patient’s timely transitional plan.
- Encourage and facilitate patient and family participation in all care team decisions.
- Collaborate with post‑acute services, care managers, and PCPs to ensure transition back to the home environment.
- Assist in adjusting daily assignments based on staffing and acuity of caseload.
- Demonstrated knowledge of NAH and department policies and procedures; works with other leadership to ensure compliance of best practice among staff.
- Establish and maintain mutual trust and respect with staff.
- Promote an environment that motivates, nurtures, and empowers staff; supports department communication lines through huddles, meetings, and other engagement means.
- Work with employees to facilitate and achieve skill, educational, and performance goals.
- Address workflow issues, ensure incidents are appropriately documented, and corrective steps are taken.
- Embrace an improvement mindset and work with leadership on process improvement initiatives.
- Advocate for both patients and staff as needs arise.
- Evaluate the appropriateness of care delivery in the inpatient setting and communicate any discrepancies with the medical team.
- Facilitate the management of care in an outpatient setting.
- Takes a leadership role in identifying opportunities to reduce risk both financial and clinical through analysis and resource consumption outcomes.
- Assertively manage resource utilization while appropriately navigating patient movement along the continuum of care.
- Stay current and comply with state and federal regulations/statutes and company policies that impact the area of responsibility.
- Ensure all certifications and/or licenses are up‑to‑date and valid prior to expiration dates.
- Complete all company mandatory modules and required job specific training in the specified timeframe.
- Report any safety related incident in a timely fashion through the Midas/RDE tool; attend all safety related training programs; perform work safely; monitor the work environment for safety issues and ensure others are also safe.
- Bachelor’s Degree or Master’s of Social Work - Required
- Master's Degree in Nursing - Preferred
- RN license in the State of Arizona - Required; or for applicants with an MSW, LMSW or LCSW - Preferred
- Case Management Certification (ACMA, CMSA, CCMC) - Preferred
- 1+ year experience in acute care management - Preferred
- Healthcare is a rapidly changing environment and technology is integrated into almost all aspects of patient care. Competence in using computers and basic software programs.
