Healthcare Nursing, Patient Care Technician
The PCT, including all hospital units and departments and the Float/Flex Pool PCT, under the supervision of the charge nurse, clinical manager, and/or director, is responsible for assisting in patient care, documentation, and supply management. PCTs keep the unit supplied, clean, and organized. Additionally, the PCT may function in an office assistant role, answering the telephone, addressing questions within their capabilities from patients or their support persons, and performing data entry and collection.
The PCT in the discharge unit assists discharged patients, coordinates discharge logistics, and helps patients utilize resources and access health-related services.
The PCT follows all policies and procedures of Northern Arizona Healthcare. They perform or assist with procedures within the scope of practice of a PCT and work closely with all members of the interdisciplinary team. The PCT may care for infants, children, adolescents, adults, and geriatrics populations.
The Float/Flex Pool PCT may be asked to work on any hospital unit, change departments during a shift, or work as a Patient Safety Attendant as needed. Call will be required for the Float/Flex Pool PCT.
This position is on-site only.
Responsibilities- Maintains department cleanliness, including procedures for post-treatment cleanup.
- Assists with patient transfers, including those with weight-bearing restrictions or motor impairments.
- Helps patients with transfers to/from wheelchairs and with activities of daily living.
- Assists with donning/doffing compression garments and removing catheters or IVs within scope of practice.
- Monitors skin integrity and reports issues to clinicians.
- Assists with phlebotomy and other procedures as within scope.
- Prioritizes and problem-solves to adapt to patient condition changes.
- Collaborates with healthcare professionals to ensure appropriate staffing and quality care.
- Assesses patient needs quickly and accurately.
- Serves as a resource for staff, assists with order entry into EMR, and supports patient care.
- Assists with dressing removal and activities of daily living under supervision.
- Supports patient mobility and safety, following all safety and infection control policies.
- Assists patients in and out of vehicles and supports discharge instructions.
- Collaborates with team and community agencies, documents in EMR, and helps secure DME as needed.
- Handles incoming calls, evaluates, and prioritizes them effectively.
- Participates in orientation and training of new staff.
- Ensures documentation accuracy, maintains charts, and assists with clerical duties.
- Documents patient data and reports abnormal findings to clinicians.
- Understands and applies standards of care, policies, and procedures.
- Engages in professional development and demonstrates competence in nursing practices in accordance with state regulations.
- Reports safety incidents promptly, attends safety training, and monitors the environment for safety issues.
- Maintains up-to-date certifications and licenses, and completes mandatory training on time.
Education
High School Diploma or GED - Required
Graduate of a Nursing Assistant Program - Required
Certification & Licensures
Current, unrestricted CNA or LNA license in Arizona - Required
Basic Life Support (BLS) - Required
Fingerprint Clearance:
If no Arizona LNA license, then a Fingerprint Clearance Card application number is required upon hire, and the clearance must be obtained within 90 days.
Employee must maintain appropriate clinical competencies as assigned.
Experience
For Discharge PCT:
Demonstrates ability to collaborate, utilize EMR, and understand post-discharge resources.
