Lead Social Worker Children/Transition/Adults Bilingual Pref #2126 — Tri-Counties Regional Center

Job Summary: The Lead Service Coordinator assists the Services and Supports Team Manager and the team in oversight of person-centered practices and assumes lead duties in the absence of the Manager. As a Service Coordinator, provides person-centered consultation, advocacy, resource information and service coordination for people with developmental disabilities who require more involved and specialized assistance. The Lead SC will have a reduced caseload to provide needed assistance to the manager and team.

Responsibilities
  • Provide training and mentoring to new employees and assist the manager in coordinating ongoing training for the team.
  • Provide person-centered training and mentoring to the team; participate in PCT meetings as an observer and provide feedback/coaching to employees. Attend local Communities of Practice and support fellow team members to do so as well.
  • Review case records for compliance with Medicaid Waiver requirements and initial annual recertification.
  • Review and approve IPP/IFSPs and IPP/IFSP amendments and the associated Purchase of Service authorizations.
  • Help organize and provide coverage when team members are out for an extended period of time.
  • Support individuals we serve by maintaining regular contact and completing IFSPs and IPPs.
  • Participate in Interdisciplinary Team (IDT) staffing and attend meetings in the community as needed, including IEP and day program meetings.
  • Provide information and referral to community services and resources (schools, recreational options, local support groups, employment programs, local agencies, and specialists).
  • Coordinate and secure TCRC funded services, including behavior services, independent living and supportive living, out-of-home and in-home respite, durable medical equipment, and specialized services (non-Medi-Cal funded as applicable).
  • Conduct all activities in a professional and ethical manner; work cooperatively as a team leader and develop relationships with community representatives.
Knowledge, Skills, and Abilities
  • Knowledge of developmental disabilities; awareness of entitlement and public benefit programs.
  • Problem-solving ability; ability to make verbal and written presentations to groups.
  • Strong organizational and time management skills; ability to work independently and within a team; responsive to supervision.
  • Ability to understand and adhere to applicable laws, regulations, policies and procedures.
  • ICF-DD experience, QMRP; ability to read/speak/write Spanish and English (preferred).
Minimum Qualifications
  • Master’s or Bachelor’s degree in a human services field or a related area, with 2 years of applicable experience in mental health or social work, and 1 year experience with individuals with developmental disabilities. 2 years Regional Center experience is preferred.
  • Reliable transportation or equivalent means of transportation; ability to travel to any location at any time.
  • Ability to read/speak/write Spanish and English (preferred).
Physical Requirements

Position requires the ability to work in a standard office setting with mobility for office equipment and computer use. This is primarily a sedentary role but may require lifting up to 20 lbs occasionally and various movements (standing, walking, reaching, bending, etc.). The job may involve working in an environment with moderate noise and exposure to upset individuals by phone or in person.
