Physical Therapist - Home Care - Full Time

Hours:

40 hours per week

Provides in-home physical therapy services to patients with neuro and/or musculoskeletal disorders, under the direction of a physician.

• Deliver in-home physical therapy services to patients with neuro and/or musculoskeletal disorders, under the direction of a physician.

• Education:
  
  Graduate of an accredited physical therapy program.
• Licensure:
  Current state license and valid driver’s license.
• Experience:
  
  Two years of experience in acute care or home care preferred.
• Certifications:
  
  CPR certification.
• Other:
  • Good assessment and treatment planning skills.
  • Energetic, good interpersonal, time management, and problem‑solving skills.
  • Strong communication and documentation skills.
  • Good physical health, able to lift up to 50 pounds frequently and occasionally >50 pounds.
  • Ability to work independently, handle emergencies without assistance.
  • Adaptability, initiative, goal‑setting, and continuous learning mindset.
  • Ability to drive in hazardous conditions, follow instructions, read a map.
  • Independent scheduling of patient visits efficiently; daily check‑in with office to update schedule.
  • Assigned weekend and on‑call rotation; regular contact with agency staff, physicians, hospital staff, and general public.
  • Handles confidential information regularly.
