  • Provide Communication and Support – Partner with patients, families, caregivers, and interdisciplinary team members to develop and follow a comprehensive care plan. Educate families on what to expect, pain management, and comfort measures.
  • Lead with Accountability – Manage your caseload independently, prioritize calls and visits with urgency, and ensure documentation is completed accurately and promptly, often at the point of care.
  • Actively Collaborate – Serve as a key member of the Interdisciplinary Group (IDG), maintaining strong communication with physicians, social workers, aides, chaplains, and referral sources.
  • Support the Whole Person – Provide clinical insight and emotional support to families before and after the patient’s death, participating in bereavement planning as needed.
  • Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
