Summary

The LPN is responsible for administering skilled nursing care to home health and hospice patients requiring intermittent professional nursing services as outlined on the patient’s individualized plan of care delegated by the RN.

Responsibilities

Creates positive experiences for internal and external customers, displaying courtesy and knowledge of services.

Assists office with achieving positive patient outcomes and provides comfort to patients/families, including performing skills and procedures, observing and reporting signs, reinforcing teaching from RN, assisting RN or physician, supporting patient family, and reporting unscheduled visit outcomes within 24 hours.

Observes and documents patient’s physical and mental health, gathering data, recognizing links to treatment, and determining need for immediate nursing intervention.

Documents electronically in the medical record, downloads patient information in Net Smart, updates medication profile, and responds to documentation requests within 48 hours.

Collaborates with supervisor to meet agency clinical needs, rotates weekend/after‑hours/holiday on‑call with RN backup, and assists with coverage of visits in other offices.

Maintains regular communication with supervisor regarding patient issues following chain of command and current technology.

Stays abreast of overall patient care and improves nursing skills through meetings, conferences, audits, quality improvement activities, in‑services, interdisciplinary team conferences, mandatory in‑services, CEU requirements, and continuing education.

Demonstrates interest in personal and professional growth for self and staff through workshops, seminars, research, and recommending opportunities.

Demonstrates willingness to be cost effective in use of agency resources, monitoring waste, and proper safe use of supplies and equipment.

Adheres to 3HC’s Personnel Policy and performs other appropriate duties assigned by supervisor.

Requirements

Licensed Practical Nurse currently licensed to practice nursing in North Carolina.
Minimum of two years of experience as a nurse in an acute care setting preferred.
Maintain current CPR certification (BCLS).
Dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
Competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the individualized plan of care.
Possess valid Driver’s License and reliable transportation.
