Position OverviewPurpose of the Position
Join us in providing primary medical management in collaboration with our esteemed PACE Organization (PO) Physicians. As a Nurse Practitioner, you'll conduct assessments, develop care plans, and ensure participants receive comprehensive care in a warm and supportive environment.

Key Responsibilities- Collaborate with PO physicians, perform assessments, and implement care plans.
- Participate in discharge planning and treat acute illnesses.
- Conduct nursing home or home visits as needed.
- Assist in formulating clinical policies and procedures.
- Participate in provider on-call rotation.
- Master's degree in Nursing.
- 5-7 years of experience, preferably in long-term care.
- Active North Carolina nursing license.
- CDS Certification, DEA Registration, and Board Certification.
- Current BLS certification.
- Ability to travel within the geographic region.
