Overview
Hope Services, LLC is seeking a Care Manager within our Tailored Care Management Department. Applicant must be a qualified professional (not licensed or provisionally licensed).
Responsibilities- The primary responsibilities of the Care Manager are:
- To promote whole-person
- Integrated care by planning
- Coordinating, tracking
- Closing gaps in care monitoring care to individuals, and achieving key goals of the tailored plan related to behavioral health, mental health, and physical health
- Must be comfortable with direct patient contact, including in-home and community appointments
- Experience with NC Tailored Care Management is required
- Applicant must be a qualified professional (not licensed or provisionally licensed)
- Experience with NC Tailored Care Management is required
The Care Manager shall report to the Care Manager Supervisor. This is a hybrid position with some in office.
