Essential Functions:

• Assist patients with activities of daily living such as personal hygiene, ambulation/transfers, dressing/shaving, and light housekeeping.
• Prepare meals and snacks according to instructions.
• Accompany patients on errands or medical appointments.
• Perform patient-specific activities taught by a nurse, including hospice supportive care, medication reminders, range of motion exercises, blood glucose monitoring, and incontinence care.
• Use lifting equipment such as gait belts and Hoyer lifts.
