$5,000 SIGN-ON BONUS FOR FULL-TIME RN CASE MANAGERS

Care IV Home Health is a locally owned and operated home health agency in Fayetteville seeking a full-time RN Case Manager to join our esteemed team. Requirements:

* Registered Nurses (RN) license that is active and in good standing with the Arkansas State Board of Nursing.

* Experience with home health nursing and OASIS is preferred; however, we will consider candidates with ICU, ER, med/surg, or related experience.

* A minimum of three professional references on the application or upon initial contact.

Duties may include, but are not necessarily limited to:

* patient education

* medication administration

* wound care

* post-surgical care

* skilled nursing care / direct patient care

* patient follow-up

* OASIS Start of Care (SOC), regular SOC, admits & discharges

* Additional duties as assigned
