Responsibilities

• Evaluate physician medication orders for appropriateness and potential interactions; recommend therapy adjustments
• Maintain patient database with medical history, lab data, and medication records
• Order and evaluate lab tests for high-risk conditions
• Collaborate with healthcare team members
• Document all communications in medical records
• Ensure safe procurement, storage, and disposal of medications
• Maintain documentation in compliance with regulations
• Report medication errors to the Pharmacy Manager
• Assist staff with medication procedures
• Perform other duties as assigned

Qualifications

• Bachelor’s Degree in Pharmacy;
  Pharm D preferred
• One year experience in long-term care
• Licenses/Certifications as required by regulations
• Attention to detail, goal-oriented, team player, good communication skills
