Overview
Optum On-Site is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our team in Waupaca, WI. As a member of the Optum Behavioral Care team, you will be an integral part of our vision to make healthcare better for everyone.

Hours:
This role requires 30 on-site hours per week – Wednesdays (7a-6p CST), Thursdays (7a-6p CST) and Fridays (7a-3p CST)
- Wellness and Preventative Physical Exams
- Acute care of minor illness and injuries
- Chronic Disease Management and Longitudinal care
- Minor procedures (suturing, splinting, incision/drainage, etc.)
- Lifestyle and well-being counseling
- Medical management of behavioral health conditions

- Certified as a Family Nurse Practitioner through the ANCC/AANP or certified as a Physician Assistant through the NCCPA
- CPR certified
- 2+ years of experience working independently providing services in primary care, family medicine, internal medicine, ambulatory, emergency or urgent care setting
- 2+ years of experience working independently providing services in primary care or family medicine

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. The salary for this role will range from $109,500 to $164,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
