About The UnitOur sub-acute floor is a 59‑bed Medicare unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac‑related issues needing monitoring. Nurses work closely with a variety of health‑care professionals including doctors, social work, dieticians, PT/OT, hospice, and spiritual support. Our sub-acute unit offers a well‑rounded opportunity for learning, professional growth and personal satisfaction.
Our Chronic Care floor is a 60‑bed Medicare/Rehabilitation and long‑term care unit and is a great place to learn basic med‑surg, I.V. therapy & wound‑treatment nursing skills. Nurses on this unit along with a host of health‑care professionals work to provide and implement the best treatment plan for residents. This fast‑paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.
Our Dementia Care floor is a 60‑bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. The dementia care unit creates a fullness of life through the joy of relationships, the art of caring and the spirit of living. This unit also works closely with a multidisciplinary team to discuss the resident’s plan of care.
About The FacilityBeaumont Commons – Farmington Hills
Our facility has 3 floors and 9 units (3 units per floor), providing care for long‑term care residents and post‑acute rehab patients. The first floor has 59 beds with a focus on sub‑acute rehab, the second floor has 59 beds focused on long‑term care and sub‑acute rehab, and the third floor houses a secured dementia unit. Nurse‑patient ratios vary depending on the shift and the population served.
With 179 beds, we accept a variety of patient specialties such as general rehab, neuro‑rehab, cardiac, respiratory and cancer. We also offer various programs for residents to participate in multiple activities. Our goal is to create a fullness of life through the joy of relationships, the art of caring and the spirit of living.
Delivers nursing care responsibly and accountably as described by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical‑thinking model known as the nursing process, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation. Upholds professional performance standards described by the ANA, including ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence‑based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health.
Strives to achieve optimal outcomes.
Required
- Associate degree ADN or equivalent
- Michigan LPN license upon hire
- BLS – AHA or ARC upon hire
Preferred
- Experience in skilled nursing in long‑term care
