Medical Coordinator
Community Living Centers, Inc. is seeking a detail-oriented, conscientious, and dedicated individual to serve as a Medical Coordinator for our Residential Group Homes.
Responsibilities
- Coordinate and monitor all resident medications, ensuring accurate medical records and timely medication administration.
- Organize and accompany residents to all medical, dental, psychiatric, and specialist appointments.
- Maintain close contact with residents regarding medical needs and stay knowledgeable of all medical conditions and prescriptions.
- Check medication deliveries and reconcile medication sheets with physician prescriptions.
- Be on call 24 hours a day for emergencies; work evenings, weekends, and overtime as required.
- Attend Person-Centered Planning meetings and provide updates on residents’ health status.
- Communicate with staff and outside agencies regarding residents’ health needs.
- Provide hands‑on care and assistance with mobility, bathing, personal hygiene, grooming, laundry, meal preparation, and mealtime assistance.
- Assist with daily living activities such as household tasks, communication skills, and community activities.
- Help develop residents’ social and recreational interests through modeling and supportive activities.
- Pass medications in accordance with DOCEBO training and physician orders.
- Transport residents using agency vehicles or personal vehicles when necessary, and maintain vehicle cleanliness.
- Respond to emergency situations and maintain timely communication with the Group Home Manager and Directors.
- Accurately complete all required paperwork and maintain residents’ medical files promptly.
- Attend and participate in training and professional development activities.
Education and Experience
- High School Diploma or GED.
- 2–3 years of supervisory or medical coordinator experience in a Group Home setting.
- 2 years of experience in the mental health field.
- Completion of all required CMH certifications and training for the counties of Oakland, Macomb, and Wayne in Michigan.
- Valid and current driver’s license with a clean driving record; must maintain insurance on personal vehicle if used for transport.
Essential Functions & Skills
- Ability to read, write, and speak English clearly; perform basic mathematical calculations.
- Comprehend and carry out written and verbal instructions accurately.
- Document accurately and completely.
- Willingness and ability to work with vulnerable adults.
- Maintain integrity and confidentiality of resident information.
Physical and Mental Requirements
- Lift, transfer, and push residents weighing up to 250 lbs; frequently stoop, twist, bend, and kneel.
- Lift supplies weighing up to 25 lbs and operate medication carts.
- Drive agency van or personal vehicle with residents as passengers as needed.
- Maintain a negative TB test and pass a satisfactory physical examination.
Additional Requirements
- Perform DSP/DCW duties as needed.
- Pass satisfactory criminal background check, abuse check, and Sex Offender Registry check.
