Responsibilities- Assessment – Collect, prioritize and synthesize comprehensive data relevant to the patient’s health or situation.
- Diagnosis – Analyze assessment data to determine and document nursing diagnoses, communicate them with the patient, family and team.
- Outcomes Identification – Identify expected outcomes individualized to the patient and collaborate to establish realistic, measurable outcomes.
- Planning – Develop individualized plans prescribing interventions to attain expected outcomes, incorporate evidence‑based practice and ensure continuity of care.
- Implementation – Safely and timely implement interventions, coordinate plan of care and document interventions per guidelines.
- Evaluation – Evaluate progress toward outcomes, involve patient/family/team, revisit diagnoses and plans, document revisions.
- Quality of Practice – Enhance quality and effectiveness of nursing practice, participate in quality improvement activities.
- Education – Acquire and apply knowledge from current practice, participate in educational activities.
- Professional Practice Evaluation – Self‑evaluate practice, identify strengths and goals for professional development.
- Collaboration – Partner with patient, family and others to effect positive outcomes.
- Ethics – Act in an ethical manner, maintain therapeutic relationships, serve as a patient advocate.
- Research – Integrate research findings and use evidence to guide decisions.
- Resource Utilization – Plan and deliver patient care using safe, effective and ethical resources.
- Leadership – Provide leadership, promote a positive work environment, participate in shared decision‑making.
- Environmental Health – Practice environmentally safe and healthy manner, communicate risks.
- Other duties as assigned.
- Associate’s or Vocational degree graduate of an accredited nursing school.
- New graduate hires must be graduate of an accredited nursing school inclusive of clinical rotations in an acute care setting (AACN or NCLN).
- Internal clinic employees who have functioned successfully as a CMA or LPN with a minimum of 3 years may be considered from an accredited school without clinical rotations.
- 0 to 2 years RN experience (may consider new graduate RN).
- Bachelor’s degree graduate of an accredited nursing school.
- Licensed Registered Nurse - MN Board of Nursing required upon hire in MN.
- Licensed Registered Nurse - WI Dept of Safety & Professional Services required upon hire in WI.
- May require both state licenses based on services at location.
- BLS Tier 1 – Basic Life Support required within 30 days.
- PEARSAH required for urgent care within 30 days.
- RN‑BC Ambulatory Care preferred.
- Intravenous Therapy (IV) Certification required within 180 days for urgent care.
- Medium Work:
Lifting up to 32 lbs occasionally, up to 25 lbs frequently. - 1.0 FTE (80 hours per two‑week pay period).
- 8‑hour day shifts (Monday‑Friday).
- No weekends (occasionally floating may be required, eligible for mileage reimbursement).
$39.19 to $53.72 per hour. Pay described reflects base hiring pay range. Shift, weekend and/or other differentials may be available to increase pay.
