Our pediatric specialty clinic is looking for an RN Navigator to join the pediatric nephrology team. This role coordinates care to ensure families and patients have the treatment, education and resources necessary to meet their needs within the Pediatric Nephrology department. The department treats patients with chronic kidney disease, congenital renal anomalies, glomerulonephritis, and related nephrology concerns. Our staff, along with the pediatric nephrologist, support outreach clinics throughout the region to increase access to specialized care.
This is a clinic setting with a variety of patients. RN navigators form strong connections with patients and families to manage their healthcare needs. We are open Monday–Friday, 8:00am–5:00pm; no weekends or holidays.
- Integrate health care for patients across the continuum of care and collaborate with the healthcare team to coordinate timely sequencing of care.
- Enhance patient flow and interdisciplinary communication; promote early intervention.
- Support patients and families by removing barriers to care and evaluating outcomes using clinical, financial and utilization data.
- Utilize independent nursing judgment in clinical integration; apply appropriate age-related patient care protocols (pediatric, adolescent, adult, geriatric as applicable).
- Serve as a key link between providers, clinics, insurance, inpatient units and outside facilities as needed.
- Establish and maintain good interpersonal relationships and work with growth and development needs of child populations.
- Coordinate, facilitate and negotiate with others; monitor and trend patient responses using structured care methodologies.
- Collaborate with ancillary and support services; solve problems and implement process improvements; work with limited direction.
- Facilitate groups and multidisciplinary teams; uphold nursing practice standards and professional conduct per state requirements and organizational policy.
- Bachelor’s degree in Nursing strongly preferred; required in Fargo and Sioux Falls locations (with variations by market).
- Graduate from an accredited nursing program (CCNE, ACEN, or NLN CNEA accepted).
- Minimum two years of clinical experience preferred; currently holds an unencumbered RN license where practice occurs (multistate license acceptable in NLC states).
- Obtains and maintains department-specific competencies and certifications as required.
- Ability to work with diverse pediatric populations and demonstrate strong communication skills.
- Approval from Nursing Executive may be required to waive bachelor’s degree in certain markets; educational plan acceptable toward degree completion within specified timelines.
