Duties of the RN - Patient Flow Coordinator include, but are not limited to:
* Management/coordination of care for Veterans admitting or discharging from the Fargo VA.
* Screening and coordinating care for admissions to the Community Living Center.
* Respite care coordination for the Community Living Center.
* Use of systems such as the Bed Management System to track patient movement.
* Provide gap coverage to Utilization Management.
* Tracking, trending and reporting patient flow related data for the healthcare system.
* Assisting with Bed Control/On-Call as needed to support the facility.
* Participate in quality improvement projects regarding patient flow.
* Coordinating Home Health Care and Hospice Care for Veterans who are discharged to assist the continuity of care coordinator.

Pay:
Competitive salary, regular salary increases, potential for performance awards
