We are delighted to offer an exciting opportunity for a Band 5 Nurse or Allied Health Professional to join our dynamic Specialist Falls Service.

The successful candidate will become part of a trust-wide team that plays a pivotal role in delivering our organisation s falls prevention agenda. This work directly supports the wider system s frailty transformation programme, making a meaningful impact across our communities.

While previous experience in falls assessment and rehabilitation is desirable, this role is also ideal for newly qualified professionals with a keen interest in this area. It offers a bespoke development opportunity, with training and supervision provided on the job.

This is a varied and rewarding role that includes:

  • One-to-one assessments and rehabilitation
  • Delivery of training and education
  • Supporting and contributing to the organisation s falls policy and governance frameworks

Main duties of the job
  • To provide clinical knowledge and support in falls prevention across the organisation.
  • Undertake specialist falls and bone health assessments for patients with complex problems, providing a clinical diagnosis where appropriate.
  • Support the specialist falls service to implement the falls and bone health pathway across the organisation.
  • To work with health and social care professionals neighbouring secondary care Trusts, local authorities, statutory bodies, and voluntary agencies to develop falls prevention strategies.
  • To undertake holistic assessments of patients who are at risk of falls and discuss with community care team, inpatient, and social care colleagues on delivery of treatment programmes.
  • To support with the line management and clinical supervision of junior and unregistered staff.
  • To support with Falls education within the organisation.
  • To carry out other duties as necessary to meet the changing needs of both the specialist falls service and of HIOW Healthcare NHS Trust, in accordance with management requirements and appropriate to banding.
