• Provide palliative care services including symptom management, education on medications and lifestyle changes to enhance quality of life, while collaborating closely with palliative specialists and community providers.
• Work with MSW to initiate discussions on advanced directives and goals of care to ensure a smooth and timely transition to hospice.
• Assess patient and family needs, report new or pertinent findings to the Palliative Specialist RN and MD.
• Follow the prescribed plan of care as completed by the RN to address identified needs.
• Demonstrate the skills and judgments to implement the medical plan of care, nursing interventions and procedures as necessary for the care of the patient, within the guidelines of your licensure.
• Evaluate and reassess identified problems, care provided and patient's response on a continuing basis.
• Identify ancillary services needed by patients and families and report findings to RN.
• Collaborate and communicate on a regular basis with members of the inter‑disciplinary team as needed.
• Assist patients and families in accessing appropriate community resources.
• Record narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and patient/caregiver response.
• All other duties as assigned.

• Graduate of an approved LPN school of nursing.
• Current LPN license in MA.
• CPR certification required.
• Minimum one (1) year recent experience in an acute care facility or the equivalent.
• Homecare and computer skills preferred.

37.5 hrs. per week, Mon-Fri, 8a-4p, Occ. Weekends & Occ. Holidays.
