Purpose of PositionProvide the highest standards of clinical support to the MACC practice. Deliver excellent customer service to patients through participation in a medical care team. Demonstrates a high level of competence with clinical support in accordance with MACC’s policies and procedures. Responsible for the management of all phases of clinical operations, including participation in the development of daily workflow within the practice.
Responsibilities- Monitor the Electronic Health Record nurse pool throughout the day, ensuring timely response.
- Triage incoming calls, messages, and inbox tasks; prioritize urgent matters and schedule appointments as needed.
- Exercise nursing judgment and standard triage guidelines to ensure health needs of patients, groups, or populations are met.
- Assess complex patient needs and transition patients to the appropriate level of care.
- Collect objective and subjective data systematically and assess the response.
- Schedule visits based on assessment of patient disposition and available resources.
- Provide clinical support to the team, adjusting role and priorities to meet patient care demands.
- Answer phone calls and electronic messages promptly and professionally.
- Greet callers with a friendly, respectful, and helpful manner.
- Deliver timely, efficient, accurate triage responses in accordance with professional nursing standards.
- Document all calls, encounters, and actions taken into the patient’s medical record in a timely and complete manner.
- Educate patients about the utilization of clinic community services.
- Collaborate with the interdisciplinary care team to avoid interruption of care delivery.
- Greets and escorts patients to exam rooms.
- Obtain relevant information and vital signs (height, weight, blood pressure, heart rate, respiratory rate, oxygen saturation).
- Input pertinent clinical information into the electronic health record accurately; perform complete medication reconciliation.
- Assist clinicians with procedures and tests, providing patient preparation information and keeping patients informed of status.
- Act as an advocate to resolve patient concerns.
- Participate in practice and department meetings and trainings to promote continuous improvement and quality patient care.
- Take initiative to learn and stay current with community resources and the broader healthcare system.
- Maintain patient rights and uphold confidentiality obligations pursuant to CCHC and HIPAA.
- Perform clerical/office support as necessary (scheduling appointments, greeting, registering patients, scanning records, faxing).
- Float to other MACC practices when required.
- Verify and implement clinician orders for testing and procedures.
- Triage walk‑in patients per established protocols.
- Maintain communication and collaboration with the interprofessional team to facilitate safe transitions and continuity of care delivery.
- Lead supervising duties: supervise and support all clinical staff; schedule staff; troubleshoot scheduling issues with the Practice Manager; ensure all policies and procedures are followed.
- Provide overall leadership in the absence of the Practice Manager.
- Lead clinical/triage staff meetings or huddles to identify improvement opportunities.
- Disseminate clinical information and workflows to the clinical team.
- Provide training and support to clinical staff and the MACC Clinical Training team as needed.
- Establish and maintain effective work relationships within the department and company.
- Challenge current practices, identify process improvement opportunities, and present recommendations to management.
- Engage in continuous improvement, aligning with CCHC Pillars of Excellence.
- Graduation from an accredited school of nursing.
- Currently licensed as an LPN or RN in the Commonwealth of Massachusetts.
- At least 2 years of nursing experience.
- Current Basic Life Support (BLS) certification.
- Ability to work in a fast‑paced environment and with all levels of personnel.
- Proficient in computer applications including the Electronic Medical Record, scheduling systems, Microsoft Office, Excel, Word, and Outlook.
- Strong communication and interpersonal skills.
- Experience with process improvement preferred.
40 hours per week, Monday‑Friday; no weekends or holidays.
