Purpose of Position
Performs a comprehensive Physical Therapy assessment of patients referred for service including but not limited to physiological, psychosocial, environmental factors and health history. Integrate pertinent data to develop a plan of care to promote function and safety. Participates with other staff in the development of agency policies and programs necessary to achieve agency objectives as they relate to patient care.
Schedule Details37.5 hrs. per week, Mon-Fri, 8 a‑4 p, rotating weekends & holidays. Area:
Plymouth.

Responsibilities- Maintain annual HHRG values at the National Standard as appropriate.
- Utilize HHRG values to establish visit frequency and appropriate interdisciplinary utilization.
- Ensure annual outcomes meet National Benchmarks.
- Refer to community resources as appropriate.
- Evaluate the clinical needs of the patient; initiate, develop põ, implement and revise HA’s plan of care; address all problems in the plan of care and document accordingly.
- Participate in inter‑disciplinary patient case conferences; evaluate the effectiveness of the established plan of care.
- Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.ائع>
- Notify MD/other involved care providers of significant changes in the patient’s condition.
- Complete admission and other paperwork promptly.
- Record narrative documentation that reflects skilled care; documentation clear, concise and addresses specifics of teaching, direct care and response of patient/caregiver.
- Obtain and clarify valid MD orders for all care delivered.
- Plan and coordinate discharges appropriately; inform patient/MD and record disposition in the discharge summary.
- Complete all discharge paperwork appropriately.
- Bachelor of Science Degree or Master’s Degree in Physical Therapy from an institution accredited by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
- Current PT license in Massachusetts with at least one (1) year of clinical experience and CPR certification required.
- Computer skills preferred.
