- Perform in‑home nursing assessments and skilled visits
- Complete OASIS start of care, recertification, and discharge assessments
- Develop and update individualized care plans
- Provide skilled nursing care including wound care, medication management, IV therapy, and catheter care
- Educate patients and caregivers
- Coordinate care with interdisciplinary team members
- Maintain accurate and timely documentation in the EHR
- Follow infection control and safety standards
- Active Virginia RN license (LPN experience may be considered)
- 2–3 Years of Nursing Experience (home health preferred)
- Experience with wound care and nursing diagnoses preferred
- Strong communication and clinical skills
- Valid driver’s license and reliable transportation
