We are seeking a Licensed Assessment Clinician for our team to ensure that we continue to provide all patients with high quality, efficient care. - Demonstrates advanced clinical skills and ability to assess patients to determine appropriate level of care.
- Identifies cultural considerations, gender identity and preferred language for patients and family
- Demonstrates advance practice in assessments, and all other documentation timely and in compliance with requirements (as appropriate to scope)
- Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition
- Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
- Recognizes and acts upon unexpected assessment findings
- Leads in ensuring a safe structured milieu with evidence based treatment modalities/resources
- Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers
- Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes
- Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team
- Develops and maintains a community resource guide including access to public assistance/alternatives
- Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope)
- Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety
- Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts
- Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families
- Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services
- Reviews, coaches and assists with training of new staff/interns as assigned within approved practice and program guidelines
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Nonviolent Crisis Intervention must be obtained within 30 days of employment start date
- Masters Degree
- (LCSW) Clinical Social Worker, or (LPC/LMHC) Licensed Professional/Mental Health Counselor - REQUIRED
