Pay range (depending on experience):
Starting at $120,000/year (PPV)

Schedule/Shift
:
Mon-Fri 8AM-5PM

Territory/Location

Fallon to Fernley, NV

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates plan of care and revises it as patient status changes.
• Regularly re-evaluates patients’ needs.
• Uses assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative nursing actions; includes patient and family in planning.
• Initiates preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed.
• Counsels patient and family regarding plan of care.
• Provides health care instructions to patient.
• Identifies discharge planning needs and implements prior to discharge.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS with hands‑on CPR certification.
• Valid driver’s license and auto insurance.
• Associate’s or Bachelor’s degree in Nursing (home health experience preferred).
