Registered Nurse – Care Coordinator

Full‑time, 35 hours per week, Monday through Friday. Hybrid remote/partial onsite schedule. No nights or weekends.

Base pay $33.00–$35.00 per hour. Sign‑on bonus $1,000.

• Provide medication reviews, both telephonic and in‑person, for acute changes of status in members.
• Review members’ medical history, medications, functional status, and need for assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as outlined in the comprehensive assessment.
• Conduct medication reviews and document them in the member’s electronic health record (EHR).
• Maintain all required documentation, including data entry into the EHR.
• Triage and balance competing priorities.
• Work independently and as part of a multidisciplinary care team.

• Registered Nurse (RN) licensed in the Commonwealth of Massachusetts in good standing.
• Experience working with youth and adults, individuals with complex medical needs, and disabled populations ages 3–64.
• Background checks (CORI and CPS) required.
