Summary
The Social Worker is responsible for the management of the social, emotional and financial wellbeing of the patients identified as needing the services of the Clinical Social Worker. The Social Worker is responsible for developing the patient discharge plan and executing this plan including coordination of the services required by the patient upon discharge. They are responsible for creating an initial appropriate assessment of the patient providing any psycho‑social intervention, as needed.
Additionally, the social worker will complete ongoing reassessments for assigned patients throughout their hospital stay.

- Teamwork:

Collaboration:
Identify high‑risk medical/social management patients, develop individualized plans with physicians/hospitalists, and work with RN Case Manager to manage clinically complex discharges. - Direct Patient Care:
Provide individualized psychotherapy, supportive counseling, and group therapy as needed; make referrals to community agencies when appropriate; discuss financial ramifications of decisions with families. - Risk Management:
Identify potential legal or risk management issues and discuss with Director/Manager; ensure resources are managed cost‑effectively while achieving positive clinical outcomes. - Education & Consultation:
Educate other health care team members and consumers; provide education/consultation to Nurse Managers and Nursing Staff; educate PCPs and clinical staff on alternative care options. - Discharge & Follow‑Up:
Document assessments, proposed plans, final discharge plans in the medical record; coordinate discharge/follow‑up activities; communicate with skilled nursing facilities, community health agencies, and other staff. - Quality & Compliance:
Attend Utilization Management Committee meetings; participate in facility Quality Improvement Plan; maintain current knowledge of regulatory requirements. - Emergency Services:
Evaluate psychiatric patients in the Emergency Department and make recommendations to the ED physician regarding disposition; complete psychiatric/substance abuse evaluations on medical floors as needed.
- At least two years of case management and managed care experience.
- Two years' experience in the clinical setting.
- Experience in developing and facilitating patient discharges from an acute care setting.
- Excellent assessment, diagnostic and crisis intervention skills.
- Master's in Social Work.
- Current Massachusetts license as LICSW.
- Certification in Case Management (preferred).

Pay Range: $66,397.24-$
