Position OverviewThe Illinois Department of Public Health is seeking a highly motivated, detail‑oriented individual to serve as a Health Facilities Surveillance Nurse in the Division of Assisted Living. This individual will conduct surveys of facilities, including state licensure and complaint surveys, prepare written survey documentation, and interview facility residents, staff, administrators, and health care providers. Under general supervision, the HFSN will travel to conduct routine health and safety surveys, observe patient care, and review medical records, policies, and procedures related to licensure and regulatory compliance.

- Under general direction, surveys Assisted Living and Shared Housing Establishments to determine compliance with state licensure requirements, and Department directives, program policies and procedures.
- Prepares written survey findings.
- Performs monitoring functions at noncompliant facilities.
- Completes necessary surveyor training.
- Provides technical assistance to facilities in emergencies.
- Participates in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed.
- Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
- Requires (a) graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience or, (b) a bachelor’s degree in nursing and two (2) years of professional nursing experience or, (c) a master’s degree in nursing.
- Requires licensure as a Registered Nurse in the State of Illinois.
- One (1) year experience in assisted living care setting.
