Primary Purpose

The primary purpose is to provide each assigned patient with routine daily nursing care and services in accordance with the patient’s assessment and care plan, as directed by supervisors.

Minimum Qualifications Education, Certification, and/or Licensure
  • High school diploma or equivalent.
  • Current Certified Nursing Assistant Certification issued by the state where services are provided, or due within three (3) months of hire.
  • WV Nurse's Aide Certification through the West Virginia Department of Health and Human Resources.
  • State criminal background check and Federal (if applicable) as required for regulated areas.
  • Obtain Basic Life Support certification within 30 days of hire date.
Experience
  • One (1) year nursing assistant experience in long term care.

Core Duties and Responsibilities
  • Record all entries on flow sheets, notes, charts, etc. in an informative and descriptive manner per policy/procedures.
  • Measure and record temperatures, pulse, and respirations per policy/procedures.
  • Demonstrate necessary knowledge and skills to provide care based on physical, psychosocial, communication, safety, education level, and related criteria for the adult geriatric patient per established age-specific standard.
  • Use equipment such as vital signs instrumentation, k-pad, scales, oxygen tanks, drainage systems/suction, SCDs, CPM machine, and bladder scanner.
  • Assist patients with daily hygiene (dental care, mouth care, bath, hair care, back rubs, nail care, shaving, etc).
  • Turn and position bedfast patient every 2 hours as needed and assist with bowel/bladder functions, bladder maintenance, and retraining programs.
  • Answer call bells promptly per policy/procedure.
  • Assist with care of the dying patient and postmortem care per policy/procedure.
  • Provide perineal and indwelling catheter care as necessary per policy/procedure.
  • Assist patients with ADL activities of daily living skills with and without adaptive equipment per instructions of therapist or nurse.
  • Serve food trays per standard, assist with feeding, record food and fluid intake, and assist patients with food arrangement.
  • Assist with lifting, turning, moving, positioning, and transporting patients into and out of bed, chairs, bath, wheelchairs, etc., using proper body mechanics.
  • Report all allegations of patient abuse and misappropriation of patient property per federal requirements per policy/procedure.
  • Report all accidents and incidents on shift per policy/procedure.
  • Assist patient with specialized feeding techniques per speech therapist recommendations per policy and procedure.
  • Assist with ambulation, gait training, and CPM administration as instructed by the physical therapist.
  • Apply orthopedic devices such as AF orthosis, abduction braces, hinge joint immobilizer per instruction of the physical therapist.
  • Use safety equipment and supplies (mechanical lifts, steady’s, etc.) when lifting or moving patients requiring more than one person assistance.
Physical Requirements
  • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling are necessary body movements utilized in performing nursing duties throughout the shift.
  • Must be able to lift, move, and position patients of all weights with varying amounts of assistance on a regular recurring basis.
  • Hearing within normal range is required.
  • Visual acuity must be within normal range.
  • Must perform heavy work:
    Exerting in excess of 100 pounds of force occasionally, up to 50 pounds of force frequently, and up to 20 pounds of force constantly to move objects.
Working Environment
  • Exposure to body fluids, human tissues, contagious diseases, sharps, and explosive gases.
  • Exposure to cleaning agents and disinfectants.
  • Exposure to toxic gas, fumes, and odors.
  • Exposure to high stress and constant interruptions.
  • Exposure to electrical current.
  • Work weekends, holidays, and all shifts.
