Hospice LPN

Join Southcoast Health as a licensed practical nurse (LPN) in our Hospice Care program. This role supports patients and families by providing compassionate, quality care in a community‑focused environment.

• Provide nursing care in line with the National Hospice/Palliative Care Organization standards and state regulations.
• Communicate effectively with the care team, patients, and families to promote goal achievement and exceptional customer service.
• Perform duties in support of the Visiting Nurse Association (VNA) supportive care program, including IV pump and central line maintenance.
• Assist with patient assessments, medication administration, and documentation using electronic health record systems.
• Participate in interdisciplinary team meetings and contribute to care planning.

• Graduate of a National League for Nursing (NLN) accredited school of practical nursing.
• One year of recent hospital or medical/surgical nursing experience required; hospice or palliative care experience preferred.
• Current LPN license in Massachusetts and Rhode Island.
• BLS and CPR certification required.
• Proficiency with electronic records and other nursing software preferred.
• Vaccinated against seasonal influenza and COVID‑19, or exempt with documentation.
